Who We Are

The CDS Wolf Foundation, named in honor of CDS Monarch founders, Lew & Phyllis Wolf – seeks donations to help support the growth and expansion of CDS Life Transitions’ community services and unfunded areas of need. Established in 2007, The CDS Wolf Foundation aims to help make a lasting impact on the lives of individuals with intellectual and developmental  disabilities, seniors, and veterans.

Residential and community living services, temporary housing for veterans, affordable housing, clinical services, case management, special olympics, recreation activities and emergency assistance are some of the unfunded areas that the CDS Wolf Foundation supports.

Our Commitment

Named in honor of Our Founders, Lew and Phyllis Wolf, the CDS Wolf Foundation seeks donations to help support the growth and expansion of CDS Life Transitions’ community services and areas of need. Gifts to the Foundation go directly to providing critically needed opportunities that provide support  to  people assisted  by CDS Life Transitions.

    Andrew Sewnauth joined CDS Monarch in 2005 when he began his career as a direct support professional and has served in numerous capacities since his start. He is the Executive Director of CDS Monarch, with responsibilities for all day-to-day operations as well as administrative functions. As Chief Executive Officer for CDS Life Transitions, he supports all affiliates through strategic leadership.

    As Executive Director of Advancement of the Wolf Foundation, David brings 23 years of not-for-profit management experience to the team. He is excited to be a part of such a diverse organization that truly makes an impact in the lives of so many.
